Here is what you can expect at your new patient appointment:
We will take the time to get to know you, and carefully review your medical history and past dental history. We will examine your teeth and take any necessary x-rays and photos to use as an aid in diagnosing any existing dental conditions. We will do an oral cancer screening and a comprehensive screening for periodontal (gum) disease. From there, we will discuss treatment options for you and we will work together to establish a treatment plan that best meets your needs.
We have made our new patient forms available to you as Adobe Acrobat files. For your convenience, we recommend printing and completing these forms at home.
Please remember to bring your completed forms with you for your first appointment. If you have any problem getting these files to print properly, please call our office and we will gladly send or fax them to you.
- New patient information form
- Completed health history form (Adult | Child)
- Signed HIIPA form
- Any current x-rays from your previous dentist
- A list of medications you are currently taking
- Written orders from your physician regarding Antibiotic Premedication
- If you have dental insurance, bring the necessary insurance cards
All patients under the age of 18 must be accompanied by a parent or guardian at the initial appointment.
Please alert the office if you have a medical condition that may be of concern prior to dental surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.)
